If you have questions, you can call us at (641)628-1121 or ask at your visit. If you cannot print and complete these forms at home, please arrive 15 minutes before your scheduled visit.

About the Visit

Your first visit to our dental office creates the foundation for a great doctor-patient relationship and for lifelong dental health. In most cases, a first appointment is a routine checkup and cleaning. We use digital x-rays for patient safety and accurate diagnostics. In addition, our preventive technology includes DIAGNOdent cavity detection and intraoral cameras.

At your visit , Dr. Allen will spend some time talking with you, asking you about your medical history and dental goals, and acquainting you with our office. He will ask what brought you to our office, whether you have any dental pain or problems, whether you feel apprehensive or stressed at dental visits, if you suffer from snoring or a sleep disorder, if your jaws pop or hurt, and whether you're interested in cosmetic dentistry.

If you have questions before, during, or after your dental visit, please ask Dr. Allen or one of our team members. The more you know, the wiser decisions you can make for the oral healthcare of you and your family.
